We make it easy for your nonprofit youth group to raise money to fund the quality programs and activities on your wish list! If you haven't placed a Fundraising Card order before, we ask that you first complete our Program Application form and send it to us via fax or mail.

Once we receive your application, we'll call to provide you with an acceptance status. Upon acceptance, we also will provide you with an account number. With that account number, you can quickly place an order for Fundraising Gift Cards!

Submit Your Account Application

By Fax:

Print out and fax your application to us at (262) 703-6500. With an ACH transaction preference, we require that you complete this form, print it out, fill in the Authorized Signature field and fax it to us with a copy of a voided check.

By Mail:

Mail your application to us in care of Credit - Gift Card Department, P.O. Box 3120, Milwaukee, WI 53201-3120. With an ACH transaction preference, we require that you complete this form, print it out, fill in the Authorized Signature field and mail it to us with a copy of a voided check.

Please allow 3-5 business days for program application processing.
